Parent/Carer Agreement

I understand that by signing this agreement and registering my child for The Curzon C of E Primary School WRAP Club, I agree to the following terms and conditions:

Admissions and Attendance

□ Places are allocated in accordance with the school's WRAP admissions criteria.

□ My child will only attend sessions that have been booked in advance.

□ I understand that attendance is subject to availability and that a waiting list may operate where necessary.

Bookings and Payments

□ All bookings and payments must be made through the Class For Kids booking system.

□ I understand that all sessions must be paid for in advance.

□ I understand that:

• Breakfast Club costs £5.00 per session, per child.
• After School Club costs £8.50 per session, per child.

□ I understand that booked sessions will be charged whether attended or not.

□ I understand that ad-hoc sessions cancelled with less than 24 hours' notice will be charged.

□ I agree to provide one month's notice if I wish to permanently withdraw my child from WRAP Club.

□ I understand that non-payment may result in my child's place being withdrawn.

Collection Arrangements

□ I will ensure my child is collected by 6.00pm.

□ I understand that if I am delayed, I must contact the WRAP Club as soon as possible.

□ I understand that repeated late collections may result in the loss of my child's place.

□ I understand that if my child has not been collected and contact cannot be made with me by 6.15pm, the school may contact Children's Social Care in line with safeguarding procedures.

Behaviour Expectations

□ I understand that WRAP Club follows the school's Relationship and Behaviour Policy.

□ I understand that children are expected to show respect towards staff, other children, equipment and resources.

□ I understand that persistent inappropriate behaviour may result in temporary or permanent withdrawal of a WRAP place.

Health and Medical Information

□ I agree to provide accurate and up-to-date medical information for my child.

□ I understand that any medication required during WRAP Club must be handed to staff and appropriate forms completed.

□ If my child has asthma, I will ensure an accessible inhaler is available during WRAP sessions.

Personal Property

□ I understand that all personal belongings should be clearly named.

□ I understand that the WRAP Club cannot accept responsibility for loss or damage to electronic devices, valuables or other personal property brought into the club.

Communication

□ I agree to provide up-to-date contact details and emergency contact information.

□ I understand that the school may contact me regarding my child's attendance, behaviour, wellbeing or any incidents occurring during WRAP Club sessions.

Safeguarding and Emergency Procedures

□ I understand that the school will take all reasonable steps to ensure the safety and wellbeing of children attending WRAP Club.

□ I acknowledge that emergency procedures, including fire evacuation and first aid arrangements, are in place and will be followed when necessary.

Acceptance of Terms and Conditions

□ I confirm that I have read and understood the WRAP Club Policy.

□ I agree to abide by the procedures and expectations outlined within the policy.

□ I understand that failure to comply with these terms may result in my child's place being withdrawn.
